Follow the links to the easy LifeCare of Florida Self-Assessment then follow these four easy steps:

1. Consult with your physician and discuss any difficulties that are limiting your independence and quality of life.
2. Ask your physician for a prescription for a therapy (physical, occupational and/or speech therapy) evaluation and treatment. A physician-signed prescription is required in order to submit for insurance payment. LifeCare can assist you in obtaining this prescription.
3. Contact LifeCare at 407-682-3600 or have your physician office fax your prescription and patient information to LifeCare at 407-682-7400.
4. After the evaluation, LifeCare will coordinate a treatment plan with your physician.
